We request advanced notice for all cancellations. Please be aware that appointments missed without prior notification may be subject to a missed appointment fee.
Why do I need to update my paperwork annually?
To maintain current and accurate records, we require annual updates to your patient and insurance information, medical/dental history, and HIPAA notices. A signed HIPAA policy is required for all patients prior to receiving treatment.
